How referral information is handled

Information we collect

The Referral & Placement Inquiry collects professional contact information and limited planning information needed for an initial housing conversation. Do not submit Social Security numbers, full birth dates, medical information, diagnoses, medication details, therapy records, safety plans, case numbers, court documents, government identification numbers, financial account information, or emergency information.

How information is used

Haven of Their Rise uses referral information solely to review the request, determine whether further conversation is appropriate, and communicate with the referring professional. Submitting a form does not guarantee placement, eligibility, or availability.

Storage and access

The referral inquiry is stored in protected website storage. Access is limited to the authorized Haven of Their Rise administrator through a sign-in-protected dashboard.

Submission confirmation

After a successful submission, the website displays a reference number. Referring professionals should save that number for follow-up.

Emergencies and sensitive records

This form is not monitored as an emergency service. Call 911 for immediate danger or 211 Texas for urgent local resources. Additional confidential documents, if later required, must be exchanged through a separately approved secure process.

Questions or correction requests

Referring professionals should retain the confirmation reference number and contact Haven of Their Rise to correct or withdraw a submission.
